Graduation Deadline - Aesthetic Fellowship

The Fellowship graduation will be held in December, 2012 at our 20th Annual World Congress in Las Vegas, NV

Aesthetic Fellowship Requirements

Modules 1-6 must be completed
Proof of current membership in the American Academy of Anti-Aging Medicine
Documentation of attendance at two or more conferences on Anti-Aging Medicine (A4M sponsored events)
Photocopy of your degree (M.D., M.B.B.S., or D.O.) from an accredited medical school
Photocopy of an active medical license in the state where the applicant resides
A CV demonstrating a minimum of five years of clinical practice experience (residency training applicable)
Signed affidavit of no disciplinary actions
12 Case Studies:
- 2 Botox/2 filler (4 for Module IV)
- 4 laser/light (4 for Module V)
- 2 chemical peel/2 sclerotherapy (4 for Module VI)
